Wellness Assessment: Each resident must have an initial wellness assessment completed prior to his/her move-In date. Services may be augmented based on each individuals personal wellness assessment and evaluation. The initial wellness assessment should include:
a. History The assessment shall be updated periodically as needs change, or whenever appropriate. Resident health status will be re-assessed when a significant change has occurred or when requested by the Administrator. Once the wellness assessment is completed, and reviewed by our staff, a joint meeting will be held with the resident and his/her family to establish the appropriate level of services. Information gathered from the wellness assessment and physician's orders is used to determine whether self-administration or assistance with medication is appropriate. Licensed staff provides health information to the Administrator to assist with the development of a service plan for each resident. Florida ALF License #7285 |
